Practice Policies

New Patient Policy

For all patients new to the practice, we require a full health history intake via a New Patient Video Intake appointment. Emails with links to complete the New Patient Intake and Informed Consent to Treat are automatically sent to the email you have on file. You must complete both forms and submit them online before your scheduled Video Intake Call.

Failure to complete and submit the forms will result in your appointment being re-scheduled via phone call, or cancelled and subject to the Late Cancellation Fee amounting to 50% of the treatment cost.

If you have not received emails to complete the forms, contact records@MidEarthAcu.com.

Cancellation Policy

Middle Earth Acupuncture believes in providing one-on-one, focused attention to your case. This means that your appointment time is reserved just for you.

A late cancellation or missed visit leaves a hole in the practitioners’ day that could have been filled by another patient. As such, we require 24-hours notice for any cancellations or changes to your appointment. Patients who provide less than 24-hours notice, or miss their appointment, will be charged a cancellation fee to the card on file.

If there is no credit card on file, the balance will be posted to your account and must be paid before or at the time of your next appointment.
